This awesome custom made bicycle jersey from the 70s has a quarter zip up the front, and two cycling pockets on both the front and back sides. It was made by the cycling clothing brand John F. Kucharik, located in Gardena, California.

This shirt fits like a modern unisex adult small. It has a tapered fit. There is a repair on the left front shoulder, and several moth holes on the back, see pics.
